In-Home Nurse Practitioner — Chronic Care & Assessments

Matrix Medical Network

Matrix Medical Network is seeking a compassionate Nurse Practitioner to provide in-home clinical assessments and health management services for health plan members and government‑sponsored program participants. In this PRN role, you will travel to patients’ residences to perform comprehensive health evaluations, chronic disease assessments, medication reviews, and preventive screenings, documenting findings in EMR and coordinating with care teams to improve outcomes. #J-18808-Ljbffr Matrix Medical Network
