Requirements:

• Board Certified or Board Eligible in OB/GYN
• Active Arizona medical license or IMLC Letter of Qualification
• Current DEA licensure required
• Minimum 1 year of OB/GYN experience
• COVID-19 and Flu vaccinations required (medical/religious exemptions and declinations considered)
• Recent case logs and consistent practice within the last 12 months preferred
