Responsibilities
- Conduct comprehensive occupational therapy evaluations for adult patients and develop treatment plans tailored to rehabilitation goals.

- Monitor progress, document outcomes, and adjust treatment plans to optimize functional recovery and independence.
- Collaborate with an interdisciplinary care team to coordinate care, progress notes, and discharge planning for safe transitions of care.
- Maintain accurate, timely documentation and ensure compliance with professional standards and regulatory requirements.

Requirements
- Ability to provide occupational therapy services within a Skilled Nursing Facility setting.
