Full-Time Registered Nurse - Home Health

Our client is seeking a Registered Nurse in Seguin, TX, and surrounding areas! If you have experience in a home health setting this opportunity could be the perfect fit for you!

Job Description

We seek a compassionate and skilled Full-time Registered Nurse to join our Home Health team in Sugar Land, Texas. As a Home Health Registered Nurse, you will play a crucial role in providing high-quality care to patients in the comfort of their own homes, promoting their health, independence, and quality of life.

Conduct comprehensive patient assessments and develop individualized care plans
Provide skilled nursing care, including wound care, medication administration, and IV therapy
Educate patients and their families on disease processes, medications, and self-care techniques
Collaborate with physicians, therapists, and other healthcare professionals to ensure optimal patient outcomes
Accurately document patient care using electronic health record (EHR) systems
Monitor and evaluate patient progress, adjusting care plans as needed
Adhere to all home health regulations, standards, and safety protocols
Participate in quality improvement initiatives and team meetings
Manage time effectively to complete all required visits and documentation within specified timeframes

Qualifications

Bachelor of Science in Nursing (BSN) required
Current Registered Nurse (RN) license in the state of practice
Minimum of 1-2 years of experience in home health nursing or related field
Strong clinical skills and ability to provide comprehensive patient care
Excellent communication and interpersonal skills
Proficiency in patient assessment, care planning, and documentation
Experience with electronic health record (EHR) systems
Current Basic Life Support (BLS) certification
Home Health Nurse certification preferred
Knowledge of home health regulations and standards
Understanding of Medicare and Medicaid guidelines for home health services
Ability to work independently and manage time effectively
Reliable transportation and valid driver's license
Flexibility to work varying schedules, including some evenings and weekends as needed
