Freedom At Home - Physical Therapy Assistant Home Health Job Summary: Freedom at Home, a Combat and Service Disabled Veteran owned and operated Small Business, is seeking a compassionate and dedicated Physical Therapy Assistant (PTA) to join our team of highly valued clinicians. As a PTA Home Health, you will provide skilled physical therapy services under the direction and supervision of a licensed Physical Therapist to patients in the comfort of their own homes. Responsibilities: Provide skilled physical therapy services under the direction and supervision of a licensed Physical Therapist Carry out individualized treatment plans designed to improve patients' mobility, strength, balance, function, and overall quality of life Assist patients with therapeutic exercises, gait training, transfer training, balance activities, and use of assistive devices Monitor patient progress and communicate changes to the supervising Physical Therapist Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques Complete accurate, timely clinical documentation Participate in case conferences, team meetings, and quality improvement initiatives Collaborate with physicians, nurses, therapists, and other healthcare professionals to provide coordinated patient care Promote safe, compassionate, high-quality care that helps patients remain independent in their homes Qualifications: Graduate of an accredited Physical Therapist Assistant (PTA) program Current Physical Therapist Assistant license in the state of practice Current CPR certification Minimum of two years of PTA experience preferred Home Health experience is a plus but not required Valid driver's license, reliable transportation, and auto insurance What We Offer: Competitive pay: $40 - $50 HOURLY Full-Time, Part-Time, and PRN opportunities Paid mileage Flexible scheduling Supportive leadership team Positive, clinician-focused culture Opportunities for professional growth The chance to make a meaningful difference in patients' lives every day Why Freedom at Home?
