Job description
Anderson Creek is a Social Rehabilitation Facility (SRF) that specializes in the treatment and rehabilitation of clients with mental health illness. The facility is developed for the purpose of providing non-medical treatment and rehabilitation services to adult mental health patients with chronic mental illness. The SRF will provide rehabilitation services that will help reduce clients’ readmissions or return to acute care facilities, hospitals, or institutions. The goal of the SRF program is to assist clients in reentering and functioning independently in their respective communities.
Under the direction and supervision of the Administrator, the Facility Manager supports the daily operation of the Social Rehabilitation Facility (SRF) by assisting with regulatory compliance, health and safety oversight, facility maintenance, rehabilitation and recreational activities, client support services, documentation, and administrative functions to ensure the delivery of quality care and services in accordance with Title 22 and Title 9 requirements.
Supervision Received:
The Facility Manager receives supervision and directions from the Facility Administrator and/or Program Director.

Duties & Responsibilities
50% - Facility Maintenance, Safety & Compliance
- Facility inspections, maintenance oversight, vendor coordination, safety monitoring, inventory management, Title 22 compliance, licensing compliance, security measures, environmental safety, maintenance reports, budgeting assistance.
Maintain facility inventory including food and supplies. - Ensure documentation and proper reporting to appropriate authorities are timely and accurate.
- Ensure compliance with licensing agencies.
- Performs maintenance, safety and management tasks that promote a clean and safe working environment for the building’s employees and clients.
- Delegating cleaning and maintenance tasks to team members
- Monitoring the safety and cleanliness of interior and exterior areas, such as living and sleeping areas, and indoor/outdoor recreation spaces
- Scheduling routine inspections and emergency repairs with outside vendors
- Ensuring proper security measures for the workplace, including collaborating with security system vendors.
- Maintaining day-to-day operations of facilities, such as completing maintenance orders.
- Creating reports on maintenance, repairs, safety and other occurrences for supervisors and other relevant staff.
- Collaborating with upper management on budgeting for facilities needs
35% – Client Supervision & Safety, Mental Health Worker Support, Rehabilitation, Activities & Documentation
- Facilitate and conduct rehabilitation activities as directed by the Administrator.
- Keep records and prepare written reports on incidents.
- Communicate the SRF rules and objectives to clients, document and immediately report noncompliance to the Administrator.
- Participate with the Administrator and other management staff in planning and implementing the services program.
- Monitor and maintain a line of sight on all clients to ensure their safety.
- Report any sign of pre-crisis behavior to the Administrator.
- Provide supervision to the Mental Health Workers.
- May participate in clients’ assessments and evaluations, treatment plan reviews, incident reporting, crisis counseling, case management support, safety maintenance, referrals, chart recording, and other duties as directed.
- May accompany clients to offsite community resources or events.
- May conduct group/activities sessions with clients.
- May take clients to out of facility appointments
10% – Meal Preparation
- Supervising meal planning and preparation activities, supporting clients in meal preparation, maintaining food inventory, ensuring food safety standards.
- Prepare meals for clients
5% – Administrative
- Participation in management meetings, service planning, employee development, special projects, and other duties as assigned.
Educational Qualifications:
- Graduation from high school or possess a GED.
Experience:
- Minimum of one year of full-time work experience providing direct care and supervision to clients with mental health illness.
Physical & Functional Requirements
- Ability to stand and walk for extended periods.
- Ability to bend, twist, kneel, reach, and lift up to 15 pounds.
- Ability to sit, write, type, and complete documentation accurately.
- Ability to communicate effectively verbally and in writing.
- Ability to respond appropriately in crisis or emergency situations.
- CPR Certification Required.
- Valid CA Driver license with good driving record (for insurance purposes)
License/Certification Required:
Must successfully pass all pre-employment clearance requirements as mandated by the California Department of Social Services (CDSS) and Community Care Licensing (CCL), including a mental and physical health screening, negative tuberculosis (TB) test results, drug screening, and a criminal background check prior to employment.
