Nurse Practitioner or Physician Assistant-(Union County, NJ) Union, New Jersey About Us Ennoble Care is a mobile primary care, palliative care, and hospice service provider with patients in New York, New Jersey, Maryland, DC, Virginia, Oklahoma, Kansas, Pennsylvania, Texas, Florida, and Georgia. Our clinicians travel to patients’ homes to provide comprehensive care for those with chronic conditions and limited mobility. We offer programs including remote patient monitoring, behavioral health management, and chronic care management to ensure the highest quality of care delivered by a trusted team. We seek individuals who are driven to make a difference and embody our motto, “To Care is an Honor.” Purpose We are seeking an experienced Nurse Practitioner to provide primary care to homebound and palliative care patients in the Union County, NJ region. You will apply medical techniques and principles under the supervision of a physician and care for a diverse set of patients with chronic diseases.
