Must-Have:
Current/very recent experience in Acute Care Adult Inpatient Hospital Setting is required.
Candidates must float within the hospital based on census.
Pre-approved time off must be included at the time of submission.
1 reference is required within the last 2 years.


Description:
The role involves care coordination for patients with medical, surgical, cardiac, vascular-related diagnoses, and some neuro conditions.
Candidates will engage in concurrent reviews, continued stay reviews, admission criteria, appeals and denials, and audits.
The patient age groups served include adolescents, adults, and geriatrics.
The daily census is approximately 20 patients.
Candidates must have experience with the EPIC documentation system.
Weekend coverage is not required, and there is no on-call requirement.
Candidates must have the ability to adapt to floating assignments based on patient census.
All candidates should be prepared to complete necessary modules prior to the start of their assignment.
Specific skills in acute hospital settings, care coordination, and managing patient admissions are essential for success in this role.
Medical and religious exemptions for flu vaccinations may be considered.
Current and relevant experience in an acute care setting is a hard requirement for this position.
