LVN Job Description and Duties in a Primary Care Clinic

In a  primary care clinic , a Licensed Vocational Nurse (LVN) provides direct patient care under the supervision of the physician, supporting the delivery of comprehensive health services.

Core Responsibilities

• Patient Assessment & Monitoring: Greet patients in a friendly manner, obtain and record vital signs (blood pressure, heart rate, temperature), height, weight, or other relevant health data.
• Plan of Care Implementation: Follow provider orders, implement and update care plans, and document all nursing interventions in the electronic health record.
• Medication Administration: Administer prescribed medications and immunizations accurately using the “Five Rights” of medication administration, ensuring proper documentation.
• Laboratory & Diagnostic Support: Collect blood or other samples and process lab specimens, perform basic diagnostic tests, and interpret results such as PPD skin tests.
• Wound & Procedure Care: Assist with pap smears, monitoring equipment, EKGs, changing dressings, removing stitches, and assist with minor procedures as directed.
• Patient Education: Explain care plans, medications, and discharge instructions to patients and families.
• Coordination & Communication: Act as a liaison between patients, providers, and other clinic staff to ensure smooth patient flow and optimal care.
• Infection Prevention: Assist in preventing, preparing for, and responding to health threats such as COVID-19, following clinic protocols.
• Inventory & Equipment Maintenance: Maintain adequate supplies, medications, and vaccines; perform equipment maintenance and control tests. Keep the patient areas and office clean and orderly.
• Supportive Services: Assist with patient scheduling, front office tasks, and administrative documentation. Must be familiar with Microsoft Office Suite.

Key Skills & Qualifications

• Current LVN license and CPR/BLS certification.
• Strong communication and interpersonal skills.
• Proficiency with EHR and medical office procedures.
• Ability to work independently and as part of a team.
• Attention to detail and commitment to patient confidentiality.
