Social Worker - Adult Medical Day Care

Overview Grace Senior Care is a growing Adult Medical Day Care dedicated to enhancing the quality of life for seniors and adults with disabilities. We are looking for a compassionate, motivated, and dependable Licensed Social Worker who is excited to grow with our company. Responsibilities Conduct psychosocial assessments for participants. Develop, implement, and update individualized care plans. Provide counseling, emotional support, and crisis intervention as needed. Coordinate with physicians, nurses, therapists, and other interdisciplinary team members. Assist participants and families with community resources, benefits, and referrals. Maintain accurate and timely documentation in accordance with New Jersey Department of Health regulations. Participate in interdisciplinary care plan meetings. Support participant admissions, discharges, and transitions of care. Ensure compliance with all Adult Medical Day Care regulations and standards. Foster strong relationships with participants, caregivers, and referral sources. Qualifications Bachelor\'s or Master\'s Degree in Social Work from an accredited program. Current New Jersey Social Work License (LSW preferred; CSW/LCSW also welcome). Experience working with seniors, disabled adults, healthcare, rehabilitation, nursing homes, or adult day care is preferred. Excellent communication and interpersonal skills.
