Site Physician - Day Treatment Center (DTC)

Location: Brooklyn, NY (Rockaway Parkway & Bushwick) Type: Full-Time | Contract with potential for temp-to-perm We are seeking a Site Physician to provide high-quality, patient-centered care within a Day Treatment Center setting serving adult and geriatric populations. This is a full-time opportunity offering a consistent weekday schedule and the potential to transition into a permanent role. Under the direction of the Regional Medical Director, the Site Physician will oversee the clinical, administrative, and organizational aspects of care delivery, ensuring excellence in patient outcomes and compliance with regulatory standards. What You’ll Do Provide comprehensive primary care to adult and geriatric patients Conduct routine exams, assessments, and follow-up care Manage chronic conditions and coordinate specialty care as needed Collaborate with interdisciplinary teams including nursing, social work, and rehab services Maintain accurate, timely documentation in compliance with Medicare/Medicaid guidelines Participate in care planning and case conferences Must-Have Qualifications Board Certified Physician (MD or DO) DEA license Experience in Geriatrics, Primary Care, or Internal Medicine Strong clinical judgment and documentation skills Nice to Have Bilingual in Spanish or Haitian Creole Experience in Day Treatment Center (DTC), PACE, or managed care settings Comfort working with medically complex, underserved populations Orientation conducted in the Bronx Coverage across Brooklyn locations (Rockaway Parkway & Bushwick) Stable weekday schedule – no nights or weekends Opportunity for long-term placement based on performance #J-18808-Ljbffr HumanEdge
