Outpatient Physical Therapist

Assignment Duration

This is a 13-week contract position.

5-8 hour days, Monday through Friday.

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with each patient's needs, goals, and physician direction. Responsibilities include the delegation and direction of all therapy services for assigned patients, ensuring the design of functional treatment programs for an effective return to community activities. This role includes educating patients, families, and colleagues while upholding the organization's mission and core values.

Responsibilities

• Complete comprehensive evaluations and develop individualized treatment plans.
• Document care in medical records accurately and in compliance with guidelines.
• Provide care under signed and valid physician orders and within insurance approvals.
• Supervise and mentor therapy students as assigned.
• Pursue ongoing professional development and continuing education.
• Understand and participate in the organization's performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience is required; new graduates are welcome.
• CPR certification.
• Active and applicable licensure for the state where work will be performed.
