Our organization is an expanding multi-specialty group with over 600 clinicians, situated approximately two hours east of San Francisco, California. The surrounding communities provide convenient access to the Bay Area and popular destinations like Napa Valley, Yosemite, and Lake Tahoe.

Position Summary

The Senior Clinics are integral to the primary care continuum, focusing on the needs of vulnerable Medicare patients under a value-based care model. These clinics address sub-acute and chronic medical issues, promote wellness and preventive health measures, and advocate for seniors, particularly those with complex needs or from underserved backgrounds. By combining clinical expertise with compassionate care, these clinics aim to enhance the health outcomes and quality of life for seniors.

The Chief of the Senior Clinic not only provides comprehensive, high-quality, patient-centered primary care for older adults but also collaborates with the local administrative leader to effectively implement the daily clinical operational model that supports holistic care.

As a vital member of an interdisciplinary team, the Senior Care Chief ensures that the collaborative processes within the clinic, facilitated through shared communication methods like huddles and team rounds, allow all care team members to contribute their insights based on their expertise and understanding of the patient. Additionally, the Senior Care Chief is responsible for ensuring that other clinicians in the senior clinic develop and maintain proficiency in comprehensive assessment and longitudinal management practices utilized across all Senior Clinics.

Key Responsibilities

• Collaborate with the Chief Medical Officer (CMO) of the Senior Clinics to uphold and enhance the integrity of the holistic clinical care model.

• Promote a patient-focused and outcomes-oriented culture where the interprofessional care model is appreciated by all team members through leadership in interdisciplinary rounds, huddles, and meetings.

• Share accountability for the clinic's financial and clinical outcomes in partnership with the local administrative leader.

• Evaluate and mentor clinicians to foster strong relationships with staff, patients, and caregivers, cultivating a team-based care culture.

• Serve as a role model for delivering care within a population health framework that emphasizes patient-centric and team-oriented approaches.

Qualifications

• MD or DO degree from an accredited institution; board certification in Family Medicine or Internal Medicine is required.

• Completion of a Geriatrics fellowship is highly preferred.

• Willingness to pursue additional training in Geriatrics.

• Possession of a valid and unrestricted state medical license.

• A minimum of 2-5 years of experience in a primary care or geriatric setting, or completion of a geriatric fellowship.

• Previous leadership experience in a primary care environment is preferred.

• Proficiency with electronic health record systems and comfort with Microsoft Office applications.

• Experience in panel-oriented and interdisciplinary clinical settings is beneficial.

• Familiarity with value-based care models is preferred.

• Proven ability to influence and lead peers through transitions.

• Exceptional clinical diagnostic, communication, and patient engagement skills.

• A genuine passion for collaborating with seniors and their families in a team-oriented environment.

Reporting Structure

• 0.2 SHN sponsored FTE reports to the CMO of the Senior Clinics.

  • Responsibilities include, but are not limited to:

    • Collaborating with the CMO regarding staff oversight, education for staff and physicians/APCs, and quality and value-based care metrics.

    • Building relationships with Ambulatory Chiefs and Chairs, as well as individual physicians, to empanel patients in the Senior Clinic.

    • Engaging with hospital leadership, hospitalists, and SNF Medical Directors to address utilization metrics (e.g., ED visits, inpatient days, SNF days).

    • Conducting outreach to patients and the broader community about the clinic's services and structure.

    • Participating in organization-wide leadership training, Senior Care-specific education, and the development of protocols, documentation standards, IDT templates, and panel management practices.

• 0.8 FTE Clinical reporting to the Medical Director of Value-Based Care and Quality, and the Internal Medicine Chair.

  • This includes direct patient care, participation in IDT meetings, and coordination with hospitalists and specialists.

Work Hours

• Traditional clinic hours, Monday through Friday.

• Shared call schedule with other Senior Clinic physicians and APCs, with an expectation of no more than one week of call every three weeks.

Compensation

• Starting base salary: $346,500.

• Potential for a $50K incentive (via SHN) based on the clinic's performance in clinical, utilization, and operational metrics aligned with system goals.

• Prorated APC supervision compensation of $12K per 1.0 FTE APC supervised.
