Position Overview: Optimize patient care transitions and resource utilization in a premier academic medical center. Lead utilization review, discharge planning, and interdisciplinary coordination for high-acuity patients while ensuring compliance with EPIC documentation standards. Why Join Us? Certification: Basic Life Support (BLS) certification is required; candidates must hold a current certification or be willing to obtain recertification prior to employment. Experience: Minimum of 1 year of recent experience (within the last 2 years) in an acute care hospital setting of similar size and complexity. Proven experience in hospital case management within an acute care environment is essential. Preferred: Previous experience in Emergency Department Case Management (ED CM) is highly desirable and will be considered a strong asset. Technical Skills: Proficiency in the EPIC Electronic Medical Records (EMR) system is required.
