Hospice Admissions RN: Care Navigation & Intake

Chapters Health System is seeking an Admission RN to assess patients at their residence or in hospitals and initiate care within our service lines. The role includes acting as an ambassador of the organization and coordinating with Care Navigation to ensure a smooth transition. The position requires an active RN license in the working state, at least 1 year of nursing experience (hospice or hospital preferred), and strong communication in English; bilingual in Spanish is a plus. #J-18808-Ljbffr Chapters Health System
