Description

• Evaluate patients in both inpatient and outpatient settings, crafting tailored treatment plans to meet individual needs
• Administer cutting-edge physical therapy interventions focused on enhancing mobility, strength, balance, and functionality
• Collaborate with an interdisciplinary team of nurses, physicians, case managers, and rehabilitation professionals to optimize patient outcomes
• Educate patients and their families on therapeutic goals, discharge plans, and effective home exercise regimens
• Document all evaluations, treatments, and progress in compliance with facility policies and regulatory standards
• Engage in comprehensive care team rounds and interdisciplinary conferences to coordinate seamless care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent accredited degree

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferable
• Certification in specialized areas (neurological, geriatric) is advantageous but not mandatory

Skills

• Prior experience is highly preferred
• Capacity to treat patients facing complex medical and mobility challenges
• Excellent communication and teamwork abilities
