Assists in monitoring the health status and providing a safe environment for participants. Functions as a member of interdisciplinary team (IDT) when needed.MISSION : Improving lives, Optimizing wellness, Promoting independenceCOMPETENCIES:Problem SolvingTeamworkEffective CommunicationResults OrientedPersonal CredibilityQuality FocusPeople FocusFlexibilityRESPONSIBILITIES AND DUTIES:ESSENTIAL JOB FUNCTIONS:Assists participants with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) as assigned or neededAssists in monitoring participants health status (vital signs, weight, skin condition, appetite and changes in functional ability). Observes participants taking prescribed medications. Reports pertinent information to Home Care Coordinator in a timely mannerAssists participants with IADLs (grocery shopping, cleaning, changing bed linens, laundry, meal preparation and other routine household care) as assignedDocuments services provided to participants consistently and accurately via Electronic Health Record (EHR). Follows procedure for flow sheet completion. Documentation is completed in a timely mannerParticipates in participants care planning through cooperation with multidisciplinary teamsHandles potentially infectious specimens with appropriate biohazard precautions, and practices universal precautionsParticipates in Company required and assigned training programs and any required staff and training meetingsOther duties as requested/assignedQualificationsHigh school diploma or equivalentMinimum of one year of experience with a frail or elderly population preferredMust have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customersMust have a current Colorado Drivers License and driving record must be in compliance with Company policy. Must maintain current automobile insurance and have reliable transportationMust be efficient with computer applications such as email, navigating the internet, using electronic timekeeping, and Electronic Health RecordsAbility to work independently with minimum supervisionKnowledge of how to use and maintain in-home equipment (adaptive, hoyer lift, etc.)
