BSW Social Worker - Skilled Nursing & Discharge Planning

Dignity Health Methodist Hospital of Sacramento is seeking a Social Worker to support residents in Skilled Nursing Care. You will conduct social work screenings and interventions to coordinate discharge planning, address medical and emotional needs, and link residents with community resources. You will plan, organize, and direct Social Services operations, interview new residents, and complete social assessments and progress notes in line with regulatory standards. #J-18808-Ljbffr CommonSpirit Health
