Title: Primary Care Physician
Location: 790 New Holland Ave B, Lancaster, PA 17602
Salary: $240,000 - $270,000 (Wider range available depending on experience and location)

The purpose of a Primary Care Physician at Oak Street Health is to provide equitable and effective value‑based healthcare to local Medicare patient populations at our innovative network of neighborhood primary care centers. Providers focus on delivering higher quality of care rather than volume, supported by large care teams. The value‑based model rewards outcomes: low hospital admissions, preventive medicine, and exceptional patient experience.

Responsibilities

• Assess and diagnose patients at local clinics
• Oversee, direct, and administer primary care
• Prescribe and administer pharmaceutical treatments and medications
• Maintain patient electronic medical records via Canopy and Greenway
• Collaborate with regional and central leadership to meet health quality goals
• Work with Practice Managers to direct and manage the center care team
• Other duties as assigned

Required Qualifications

• Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.)
• Graduate of Internal Medicine or Family Medicine, Board Certification (or board‑eligible)
• Active, non‑probationary, unrestricted State License
• Active DEA license
• US work authorization
• Embodies “Oaky” values

Preferred Qualifications

• Fellowship training in Geriatrics
• Experience practicing medicine with geriatric populations
• Experience operating in a value‑based health‑care model
• Experience in a collaborative setting ensuring positive health outcomes
• Experience in outpatient primary‑care settings
• Bilingual proficiency (where applicable)

Pay range: $174,070.00 – $374,920.00 (base salary dependent on experience, education, geography, and other factors)
