Location: UBMD Primary Care – 77 Goodell Street, Buffalo, NY 14203 (This position may be eligible for a remote/hybrid work arrangement following the completion of training and must be available to attend on-site meetings as required.) About the Role: UBMD Primary Care is seeking a detail-oriented LPN Quality Specialist to support quality improvement initiatives and patient care coordination across our primary care practices. This role is ideal for a licensed practical nurse who is passionate about improving patient outcomes, advancing quality measures, and collaborating with care teams to enhance efficiency, satisfaction, and overall healthcare delivery. Following a successful training period, this position may be performed remotely, with availability to attend on-site meetings and activities as needed. What You'll Do: In this role, you'll leverage your clinical knowledge, quality improvement expertise, communication skills, and patient-centered approach to support care teams and improve outcomes for patients. Quality Improvement & Care Coordination Skills: Partner with care team members to meet and maintain quality measures established by payers and regulatory programs Support practice redesign initiatives and workflow improvements Conduct pre-visit planning to ensure providers have necessary patient information before appointments Perform transitional care outreach to patients recently discharged from hospitals or emergency departments Reconcile medications and assist patients with post-discharge follow-up care Answer patient questions regarding the transition of care process Monitor and document practice progress toward quality and performance objectives Support quality improvement projects and practice transformation efforts Facilitate efficient and productive improvement team meetings Assist practices in utilizing health information technology tools to improve patient outcomes and office efficiency Clinical & Patient Care Skills: Coordinate timely follow-up for prescription requests, referrals, diagnostic testing, and sick visit appointments Collaborate with healthcare providers, community agencies, patients, and family members regarding care coordination Provide patient education materials and community resource information Support population health management initiatives Promote patient-centered care principles and best practices Independently gather patient information and resolve care coordination concerns when patients are unable to communicate directly Support Patient-Centered Medical Home (PCMH) standards and initiatives Operational & Support Skills: Work collaboratively with the Quality Improvement team under the direction of the Director of Quality Management Pilot and evaluate new workflows and approaches to patient care Assist with goal setting, action planning, care management, and patient safety initiatives Provide guidance and support to residents and medical students participating in quality improvement activities Connect practices with external resources, educational opportunities, and consultants when appropriate Review and address daily assigned tasks Review and respond to company email daily Maintain compliance with HIPAA, OSHA, and confidentiality requirements Travel to other UBMD Primary Care locations as needed Perform additional duties as assigned by leadership Skills That Lead to Success: Core Skills: Strong communication skills with patients, providers, leadership, and healthcare teams Excellent organizational and multitasking abilities Attention to detail and accuracy Ability to manage multiple priorities independently Critical thinking and problem-solving skills Professionalism, reliability, and adaptability Commitment to patient-centered care and quality improvement Technical Skills: Quality measure tracking and performance improvement Pre-visit planning and care coordination Transitional care management and hospital follow-up Population health management tools Electronic Medical Record (EMR) proficiency Patient education and resource coordination Knowledge of Patient-Centered Medical Home (PCMH) principles preferred Standard office technology and software applications Qualifications : Graduate of an accredited nursing program or certified LPN program Current and valid New York State LPN license required Minimum of two (2) to three (3) years of experience in an outpatient clinic or primary care setting preferred Experience working with quality measures and quality improvement initiatives preferred Knowledge of Patient-Centered Medical Home (PCMH) standards preferred Experience providing patient education preferred Excellent communication, organizational, and multitasking skills required Commitment to ongoing professional development and continuing education Work Environment: Fast-paced, well-lit office and healthcare environment Mild to moderate office noise levels Moderate indoor temperatures Potential exposure to occupational health hazards in clinical settings Occasional travel between practice locations may be required Extended periods of sitting or standing while working on a computer Close computer work and frequent use of office technology Light lifting up to 5 pounds Regular, predictable attendance required Ability to communicate effectively with patients from diverse backgrounds Additional Information: Employment is contingent upon a successful background check and drug screen.
