Occupational Therapist (OT) - PRN Location: Colonial Nursing and Rehab - Crown Point, IN Colonial Nursing and Rehab is seeking a compassionate and dedicated Occupational Therapist (OT) to join our skilled nursing and rehabilitation facility in Crown Point, IN. In this role, you will deliver high-quality therapy services designed to improve residents' independence, function, and overall quality of life. Position Details

- PRN
- Implement individualized treatment plans developed in collaboration with the rehabilitation team
- Conduct thorough evaluations and establish measurable short- and long-term therapy goals
- Educate residents, families, and caregivers on techniques that support ongoing progress
- Work closely with interdisciplinary team members to coordinate comprehensive care
- Monitor resident progress and adjust treatment plans as appropriate
- Maintain accurate and timely clinical documentation
- Active Occupational Therapist license (OTR/L) in Indiana or associate's degree from an accredited OT program
- Commitment to patient-centered care
- Strong communication and interpersonal skills
- Ability to work effectively as part of an interdisciplinary team
- Previous skilled nursing or rehabilitation experience preferred
