Street Outreach Specialist

Job Summary We are looking for a Street Outreach Specialist to find, connect with, and enroll eligible individuals into our Enhanced Care Management (ECM) and Community Supports (CS) program. This role focuses on building trust and guiding people toward a system of support designed for their success. Duties/Responsibilities Drive Enrollment & Conversion: Manage the full engagement cycle from contact to onboarding. Generate Leads & Build Your Pipeline: Proactively canvass communities to identify and engage potential members. Qualify Prospects with Precision: Master the DHCS eligibility criteria to assess qualification accurately. Build a High-Value Referral Network: Forge relationships with partner agencies, community leaders, and organizations. Maintain Your CRM & Report on Metrics: Track all interactions, conversions, and progress in our systems. Perform other duties as assigned. Education and Experience High School Diploma or GED required. At least one year of experience in direct outreach, sales, or community engagement. Direct field experience working with unhoused or other high‑need populations highly desirable. Required Skills/Abilities Excellent verbal and written fluency in English. Bilingual skills, especially Spanish, desired. Knowledge of community and county resources. Proficiency with laptops, smartphones, Microsoft Office, and email. Street‑smart and empathetic ability to navigate complex social landscapes. Sales and engagement drive to meet and exceed enrollment targets. Valid California driver license, personal auto insurance, and MVR sufficient for agency auto liability. Compensation & Incentives Competitive base salary and performance‑based bonus structure. Commission on enrollment with uncapped potential. Tiered achievement bonuses for milestones. Base salary provides a stable foundation. Physical Requirements 75% of work conducted in community; 25% office. Can lift minimum 15 lbs. Prolonged periods of sitting at an office desk on the computer. Pay range: $25 USD – $28 USD.
