Clinical Pharmacy Specialist

Pharmacist

Provides specialized clinical activities relating to designated clinical programs in order to ensure optimal patient outcomes. Performs various types of medication utilization activities within a defined area including but not limited to anticoagulation pharmacokinetics dose adjustments order verification and IV to PO-conversions. Participates in committees and represents the department of pharmacy throughout the hospital or ambulatory settings within the area of focus.

Retention Bonus: $10000 - 1 yr of experience required

Education:

• Doctorate
• Pharmacy

Experience:

• Three Years

Credentials:

• Pharmacist-LIC
• Basic Life Support
• Board Certified
• Residency
