Your Role

• Complete in-home physical therapy evaluations
• Create and deliver customized therapy plans
• Provide one-on-one sessions tailored to each patient's goals
• Educate patients and families on exercises for continued improvement
• Collaborate with nurses, physicians, and other care providers
• Document patient progress accurately

What You Bring

• Valid Illinois Physical Therapist license
• Strong assessment and clinical skills
• Compassion, empathy, and great communication abilities
• Reliable transportation to patient homes
• Home health experience preferredbut we'll provide support if you're new to it
