Job Title: Registered Nurse (RN) – Home Care Job Type: Full-Time Location: Philadelphia County, PA (In-Person)

What You’ll Do:
- Conduct comprehensive initial assessments of patients and their families to determine care needs.
- Develop and regularly update individualized care plans that reflect patients’ evolving conditions.
- Implement preventive, therapeutic, and rehabilitative nursing procedures in the home setting.
- Administer prescribed medications and treatments safely and accurately.
- Collaborate closely with physicians, caregivers, and other health professionals to coordinate care.
- Supervise and evaluate care provided by home health aides every two (2) weeks.
- Participate in the rotating on-call schedule as needed.

What You’ll Need:
- Graduate of an accredited nursing program
- Active Registered Nurse (RN) license in Pennsylvania
- Current CPR certification
- Valid driver's license and reliable transportation
- 1–2 years of acute care experience in a healthcare setting
- Preferred: Bachelor's degree and prior experience in home health care
