Job Title: Registered Nurse (RN) – Home Care Job Type: Full-Time Location: Philadelphia County, PA (In-Person)

What You’ll Do:

Conduct comprehensive initial assessments of patients and their families to determine care needs.
Develop and regularly update individualized care plans that reflect patients’ evolving conditions.
Implement preventive, therapeutic, and rehabilitative nursing procedures in the home setting.
Administer prescribed medications and treatments safely and accurately.
Collaborate closely with physicians, caregivers, and other health professionals to coordinate care.
Supervise and evaluate care provided by home health aides every two (2) weeks.
Participate in the rotating on-call schedule as needed.

What You’ll Need:

Graduate of an accredited nursing program
Active Registered Nurse (RN) license in Pennsylvania
Current CPR certification
Valid driver's license and reliable transportation
1–2 years of acute care experience in a healthcare setting
Preferred: Bachelor's degree and prior experience in home health care
