Day Habilitation Coordinator/QIDP - Case Manager

The Day Habilitation Coordinator / Qualified Intellectual Disabilities Professional (QIDP) – Case Manager is located in the Schenectady Site‑Base Day Habilitation in Schenectady County. Job Type: Full‑time. The Day Habilitation Coordinator, QIDP, is responsible for the development and review of all Day Habilitation Plans and Outcomes at our Day Community Opportunities Program. The Coordinator ensures outcomes are carried out according to the Day Habilitation Plan and acts as an advocate for all individuals enrolled. Job Responsibilities Develop and write Day Habilitation plan for each Participant. Rewrite the Day Habilitation plan to reflect changes. Coordinate and review the Participant’s plan with the family, Service Coordinator, and Participant on a semi‑annual basis to discuss progress/regression and overall performance within Day Habilitation. Communicate with the Participant and/or provider to ensure that the Day Habilitation plan of service meets the Participant’s needs and desires; supply guidance or direct intervention when it does not. Requirements Bachelor’s Degree in human services. One (1) year of experience in providing services to persons with developmental disabilities. Good writing, typing, and interpersonal communication skills. Valid NYS driver’s license and reliable transportation. Benefits Paid Training.
