Job Type Full‑time Role Overview Benchmark Human Services is hiring Case Managers in Vermont to provide resources and support to individuals with intellectual and developmental disabilities and to their families and guardians. The role supports individuals to be included in society, live independently, and function at their maximum potential. Perform evaluations and assessments to meet the needs of individuals served. Assist and support the individual in understanding the goals and activities of the person‑centered planning process, including assessment. Facilitate and schedule participation in person‑centered planning and assessment as needed. Complete the Needs Assessment Annual Review Protocol once a year with the individual and their team. Assist the individual in discovery and exploration discussions, identifying preferences, strengths, goals and dreams, and needs in preparation for person‑centered planning. Facilitate the completion of the DS Budget and Service Planning Context Document. Facilitate additional assessments and gathering of documentation as needed, ensuring this information is available for the planning process. Assist the individual in selecting people to be part of the planning team who may provide valuable information. Assist the individual in inviting other people of the individual’s choosing (excluding legally prohibited individuals) to participate in planning meetings at times and locations convenient for them. Work with the individual to develop agendas and follow‑up notes for planning meetings and distribute this information to team members as approved. Support the individual to lead, and provide assistance as needed, throughout the planning process. Assist the individual and their support team in identifying any needed modifications to rights and freedoms. Review and update the person‑centered service plan and related documents (including assessments), with the individual. Monitor service plan implementation, satisfaction, and health and well‑being through visits, document reviews, and engagement with guardians, family members, and service providers. Review, assess, and respond to emergency situations and incidents, ensuring actions are appropriate and timely to protect the health and welfare of individuals. Validate that services provided are consistent with the ISA/BIP person‑centered service plan and the participant’s needs and desired outcomes. Knowledge, Skills, and Abilities Knowledge of Vermont’s public service system for developmental disabilities and brain injury services. Fundamental understanding of state statutes, rules, definitions, program requirements, and specific services, variances, and exceptions. Ability to work collaboratively while meeting individual performance goals. Ability to communicate verbally and non‑verbally with a broad audience of stakeholders. Ability and skill to use technology tools in daily duties. Qualifications Valid driver’s license. Valid auto insurance. Must live in the state of Vermont. Minimum of one year working in a similar position. Travel up to 75 %. Knowledge of Vermont’s public service system for developmental disabilities and brain injury services. Bachelor’s degree in any field + 1 year of relevant human services experience (e.g., supporting individuals with disabilities, special education, sociology, vocational rehabilitation, protection and advocacy, psychology, substance use, aging, healthcare, social work, or related field). OR associate degree in human services or a closely related field + 2 years of relevant experience. OR high school diploma or equivalent + 3 years of relevant experience.
