Registered Nurse - Case Management - Hospital

Job Summary:

Provide specialized case management nursing services within a hospital setting to facilitate patient care coordination, discharge planning, and resource management. Ensure optimal patient outcomes through assessment, planning, and collaboration with multidisciplinary teams.

Responsibilities:

• 
  Assess patients clinical needs and develop individualized care plans.

  Coordinate patient services and facilitate communication among healthcare providers.
  
  
  
  
• 
  Monitor patient progress and adjust care strategies accordingly.
  
  
  
  
• 
  Educate patients and families on treatment plans and post-discharge instructions.
  
  
  - Document all patient interactions and care activities accurately using electronic health record systems.
  
  
  
  
• 
  Collaborate with medical staff and social workers to ensure timely discharges and resource utilization.
  
  
  
  
  
  
  
  
  Qualifications:
  
  
  
  
• 
  Valid RN license in the state of practice.
  
  
  
  
• 
  Minimum of 2 years experience in an acute care or hospital environment.
  
  
  
  
• 
  Strong knowledge of case management principles and hospital workflows.
  
  
  - Excellent communication and interpersonal skills.
  
  
  - Ability to multitask efficiently in a fast-paced setting.
