365 Healthcare is seeking a Medical Director – Geriatric Medicine Physician for a full-time, permanent opportunity in Fayetteville, NC.

Lead a PACE (Program of All-Inclusive Care for the Elderly) delivering comprehensive, community-based care focused on keeping seniors independent and out of nursing homes.

Position Highlights

• $250,000 – $290,000 base salary
• Monday–Friday schedule (daytime hours)
• Leadership + clinical role
• Shared on-call rotation
• Full-time permanent position

Practice Overview

• Established PACE program serving 55+ population
• Interdisciplinary team model (MDs, APPs, nurses, social workers)
• 200+ participants with program growth
• Care provided across clinic, home, and community settings
• Strong administrative and clinical support

Responsibilities

• Provide medical direction and clinical oversight
• Supervise physicians and advanced practice providers
• Lead quality initiatives (QAPI)
• Manage complex geriatric patients
• Participate in care planning and interdisciplinary meetings
• Support program development and growth

Requirements

• Board Certified – Internal Medicine or Family Medicine
• Geriatric experience required
• Geriatrics certification preferred
• Active NC license (or ability to obtain)
• DEA & CDS required
• Leadership experience required
