PT - Physical Therapist

Home Health Physical Therapist/Occupational Therapist/Speech-Language Pathologist

This is a 13-week contract position.

Monday to Friday, 8-hour shifts.

We are seeking experienced Home Health Physical Therapists, Occupational Therapists, and Speech-Language Pathologists to provide high-quality care in Lehigh and Northampton counties. Candidates may be asked to assist in other territories within a 60-mile radius of the facility address as needed.

Responsibilities:

• Conduct patient evaluations and assessments.
• Implement and modify treatment plans based on patient needs.
• Document patient progress in Homecare Homebase and OASIS.
• Coordinate with the interdisciplinary team for comprehensive patient care.
• Manage a productivity benchmark of 28-32 points per week, handling 4-6 visits per day based on visit type.

Required Experience / Certifications / Licensure:

• Home Health and OASIS experience is required, with no exceptions.
• Experience with Homecare Homebase documentation required.
• Contract may consider candidates without HCHB experience at a reduced rate for the first two weeks if the manager is willing to provide training.
