Requirements:

• 2 years of experience as an RN
• Valid RN license
• Be willing to obtain Tennessee licensure
• Experience with OASIS and Homecare Homebase EMR systems

Responsibilities: As a healthcare professional in a Home Health setting, you will play a vital role in providing compassionate patient care, including conducting initial assessments with an RN, managing patient medications, and coordinating with healthcare teams to ensure quality outcomes.

• Assess and develop individualized care plans as an RN, ensuring compliance with client protocols.
• Utilize OASIS and Homecare Homebase EMR systems to document patient progress accurately.
