RN - Case Management

Monday to Friday, 8‑hour shifts (8A – 5P) Job Description We are seeking a compassionate and dedicated Case Management Registered Nurse to join our team. The ideal candidate will have expertise in discharge planning and observation management, with a focus on providing exceptional patient care. You will collaborate closely with multidisciplinary teams to ensure smooth transitions and continuity of care. Responsibilities Conduct assessments for discharge planning and patient needs. Manage observation cases effectively following established protocols. Coordinate care with healthcare teams and community resources. Document patient progress and care plans in Midas, Meditech, and Well Sky. Ensure compliance with interqual criteria and hospital policies. Required Experience / Certifications / Licensure Current RN license in the state of practice. Experience in case management and discharge planning is essential. Advanced certifications in case management preferred.
