Nurse PractitionerCarease Health is seeking an experienced Nurse Practitioner (NP) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. As a Nurse Practitioner with our organization, you will have the opportunity to work in a hybrid role that combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.As a key member of our clinical team, you will be responsible for building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player.
