Primary Work Location Natalie’s Place is one of several Cardea Health transitional housing programs for medically complex and behaviorally vulnerable unhoused individuals, located in Fremont. The program collaborates with a wide range of referring partners across Alameda County to support individuals with complex health and social needs. Services include onsite nursing, caregiving support, transportation to medical appointments, nutritious meals, housing navigation, placement assistance, and additional wrap‑around services. Schedule Rotating days, weekends included Job Summary Cardea Health seeks a highly motivated Registered Nurse with a special interest in working with people experiencing homelessness. The RN will serve as the primary point of clinical care at Natalie’s Place. The ideal candidate is invested in Cardea Health’s mission, demonstrates flexibility, patience, and the ability to work in challenging environments with a diverse population. Experience with homeless or incarcerated populations is highly desirable. Job Duties Provide quality care in accordance with Cardea Health policies, procedures, and standards. Ensure all clients are treated with dignity and respect. Conduct client intake assessments upon arrival and identify clinical needs and individualized goals of respite stay. Execute care plans for admitted clients in collaboration with primary and specialty care teams, medical directors, and clients. Provide clients with appropriate information and education to make informed choices related to medical care and treatment. Maintain communication among members of the clinical team and with partner hospitals, community health clinics, homeless service providers, and other collaborating agencies. Evaluate and triage referrals to the program and communicate with referring partners about referral status. Work with team members to develop creative solutions to meet the needs of individual clients. Participate in the collection and maintenance of data on program operations for reporting and analytical purposes. Ensure adherence to infection control standards on site, including Covid‑19 prevention measures. Ensure clinical documentation is current, timely, and accurate. Maintain confidentiality in accordance with HIPAA. Recognize and respond to medical emergencies, following up with MD communication or 911 activation. Practice harm reduction principles, intervening to minimize harmful effects of substance use while treating all residents with dignity and respect. Other duties as assigned. Constant work around and/or with people. Interaction with patients, clients, and external customers. Clinical setting in a respite supportive housing site. Minimum Required Skills & Qualifications Associate or bachelor’s degree in nursing. Current license as a Registered Nurse through the California Board of Registered Nursing. Current Basic Life Support Certification. Personal, reliable transportation to and from work sites and shifts. Must be 18 years of age or older. Must provide proof of COVID vaccination or be willing to be vaccinated. Preferred Skills & Qualifications Two years of experience as a Registered Nurse. Self‑directed and able to work with minimal supervision. Detail oriented and highly organized. Prior experience with homeless or incarcerated populations is desired. Proficiency in Microsoft Outlook, Word, and Excel. May require evening, holiday, and weekend hours. Physical Requirements Frequent sitting or stationary work. Frequent standing, walking, kneeling, stooping, and ascending stairs. Frequent lifting and carrying up to 20 lbs. Frequent pushing and pulling. Frequent reaching high, low, and level.
