As a Nurse-Family Partnership Public Health Nurse, you will partner with first-time parents across Kitsap County, providing vital support during pregnancy and the first two years of their child's life. The Position: Under the direction of the Nurse-Family Partnership (NFP) Public Health Nurse Supervisor, in coordination with the Parent Child Health Program Manager, and within state and agency policies, you will provide comprehensive nursing services to clients and families eligible for participation in the NFP program. In this role, you will maintain the highest standards in clinical nursing practice and fidelity to the NFP model, adhering to policies, procedures, guidelines, and standards of both NFP and the agency. This position requires advanced decision-making strategies, independent judgment, and a strong sense of accountability. You will work extensively with socio-economically and culturally diverse client populations, partnering closely with medical professionals, community organizations, and other District staff to provide NFP services. Hours: 32 hours per week – 80% FTE, requires occasional evening and weekend hours. Status: Regular - Permanent part-time position Benefits: The District offers a comprehensive health benefit plan that includes medical, dental, vision, and life coverages, including several voluntary plans through The Standard, MetLife, and Aflac. The District provides medical and dental coverage, contributing percentages of the average medical and dental insurance premium cost for full‑time employees as follows: Employee @ 100%, Employee & Spouse @ 90%, Employee & Child(ren) @ 90%, and Full Family @ 85%. Benefits include generous paid time off and twelve paid holidays per year. Employees participate in the Public Employees Retirement System (PERS), a defined benefit retirement plan managed by the Washington State Department of Retirement Systems and Social Security. Employees also have access to several 457 deferred compensation plans. Benefits for part‑time employees are prorated based on full‑time equivalence. Upon hire, core training and support for implementing the Nurse Family Partnership model of care is provided by Changent. After an initial trial service period with sustained, satisfactory performance, generally at least three months, a limited hybrid remote work arrangement may be established as mutually beneficial.

Bilingual (Spanish or Mam) preferred.ESSENTIAL FUNCTIONS
Position‑Specific Functions: Provides home, office, telephone, and/or video conferencing visits to NFP clients and their families. Maintains fidelity to the NFP model of home visitation. Carries a caseload of up to 25 clients (caseload is prorated based on full‑time equivalence). Supports policies, procedures, guidelines, and standards of NFP and of the District. Promotes public awareness of NFP. Supports program developments, assists in grant oversight and reporting, and meets grant deliverables as assigned. Develops and maintains community relationships to support client referrals. Completes all required NFP education. Successfully maintains reliability with the use of the Dyadic Assessment of Naturalistic Caregiver‑child Experiences (DANCE) assessment tool. Develops therapeutic relationships with clients and their families in a home visiting environment. Performs home visiting in accordance with the NFP model and guidelines. Assesses physical, emotional, social, and environmental needs of clients and their families as they relate to the NFP domains. Assists clients and their families in establishing goals and outcomes. Provides education, support, and referral resources in assisting clients and their families with attaining their targeted goals. Consults and collaborates with other professionals involved in providing services to clients and families. Actively engages in skill building to meet all NFP Nurse Home Visitor competency requirements. Meets with the NFP Nurse Supervisor weekly for clinical supervision. Utilizes reflective practice in supervisory sessions. Schedules joint home visits with NFP Nurse Supervisor every four months. Attends and participates in weekly case conferences and staff meetings. Provides information to support staff for timely and accurate data input to the NFP ETO database. Cooperates in review and analysis of the ETO reports for achievements and areas for improvement; participates in quality improvement efforts. Documents nursing care using the Omaha System standardizes documentation language and the CHAMP electronic health records, Nightingale Notes, within 48 hours of the nursing visit. Understands, supports, and coaches others in the NFP vision, mission, and model. Represents the NFP vision, mission, and model in actions and verbally to both internal and external customers and colleagues. Supports program development and planning for pregnant clients, children, and families in collaboration with the Parent Child Health team, other District employees, community partners, and families. Assists in creating a positive work environment that promotes productivity, mentoring, teamwork, and cooperation; supports others in accomplishing goals and objectives. Elicits and considers differing viewpoints when analyzing issues. Maintains clear, effective, open, honest communication with both internal and external customers and colleagues. Creates, maintains, and supports a safe environment for open discussion. Seeks and responds appropriately to feedback. Accurately assesses own learning needs and develops strategies to meet them. Stays informed on current health care developments to provide safe, quality nurse home visiting services. Establishes and records progresses toward annual goals. Participates in and contributes to ad hoc special activities and assignments. Keeps current on and complies with HIPAA and other privacy regulations and policies, maintaining confidentiality at all times. Prepares daily logs, bills, monthly reimbursement requests, and reports as needed. Attends scheduled staff meeting to keep current on procedural changes. May provide cross‑coverage for all other clinical and nursing services as needed. Completes timely and accurate random moments to comply with contractual requirements of Medical Administrative Match. General Agency‑Wide Functions: Accurately completes and submits electronic timecards on a weekly basis. Maintains accurate and organized records, databases, systems, and files. Prioritizes and plans own work activities. Uses work time and resources effectively. Prepares a variety of letters, memos, forms, reports, and other documents; operates computers utilizing various software applications to produce materials with clearly organized thoughts using proper sentence construction, punctuation, and grammar. Responds to public health emergencies as required by the District. Participates in quality improvement efforts. Serves as a resource person for staff in their area of expertise. Demonstrates cultural humility and embeds health equity into all aspects of assigned work and interactions. Works both independently and within a collaborative team-oriented environment; contributes openly, disagrees respectfully, understands the ideas of others, listens well, and works for consensus. Models an organizational culture rooted in respect, transparency, and collaboration, utilizing data‑informed management to drive continuous improvement. Establishes and maintains cooperative, effective working relationships with other District employees and the public using principles of good customer service. Participates in and contributes to staff meetings, ad hoc committees, special activities, and assignments. Reports for scheduled work with regular, reliable, and punctual attendance. Performs other duties as assigned.WORK ENVIRONMENT & PHYSICAL DEMANDS
Work is performed both indoors in an office environment and in the field (e.g., conducting home visits). Requires the ability to communicate with others orally, face to face and by telephone, and via virtual platforms such as Zoom, Microsoft Teams, etc. Requires manual and finger dexterity and hand‑eye‑arm coordination to write and to operate computers and a variety of general office equipment. Requires mobility to accomplish other desktop work, retrieve files, and to move to various District locations. Requires visual acuity to read computer screens, printed materials, and detailed information. Essential duties may involve occasional kneeling, squatting, crouching, stooping, crawling, standing, bending, climbing (to stack, store or retrieve supplies or various office equipment). May occasionally be required to work a varying schedule which may include evenings and weekends. Duties require carrying a cell phone or electronic device. May be exposed to individuals from the public who are upset, angry, agitated, and sometimes hostile, requiring the use of conflict management and coping skills. Frequently required to perform work in confidence and under pressure for deadlines, and to maintain professional composure and tact, patience, and courtesy at all times. The environment is dynamic and constantly changing, resulting in continually re‑evaluating and shifting priorities. The incumbent is required to use the combined senses of touch, sight, smell, and hearing during ongoing client assessment. Duties require the ability to read typewritten and/or handwritten orders and notes. Off‑site visits require the incumbent to carry supplies and/or equipment up and down stairs as well as into and out of a vehicle on a daily basis. While performing home visits, the incumbent may encounter unsanitary conditions, animals (pets) and clients and/or family members who are smoking, exposing the incumbent to second‑hand smoke which may cause allergic reaction and/or illness. The incumbent may be exposed to emotionally disturbing experiences. The incumbent is expected to remain calm, controlled, and professional, regardless of the situation and to demonstrate care and compassion to the client, family member and other members of the health care team. While conducting home visits, the incumbent may be exposed to potentially violent clients and/or family members who may be under the effects of alcohol or drugs, are mentally unstable or experiencing homelessness. As a professional obligation, the incumbent is required to report abuse if it is encountered, which may put additional stress on the incumbent. At times, the emotional state of family members brings uncertainty in knowing what to expect while at work. As a result, there is legitimate concern about one's own safety as well as being responsible for the lives of patients and families, risk of assault and unknown and unpredictable situations. Regularly, the incumbent will be required to lift and/or carry objects and materials up to ten pounds (taking infant scale into patient homes during a visit). Occasionally, the incumbent may be required to lift and/or carry objects up to twenty‑five pounds. May be required to stay at or return to work during public health incidents and/or emergencies to perform duties specific to this classification or to perform other duties as requested in an assigned response position. This may require working a non‑traditional work schedule or working outside normal assigned duties during the incident and/or emergency.EDUCATION & EXPERIENCE
A bachelor's or master's degree in nursing (BSN or MSN) with coursework that includes a community health component. OR A bachelor's degree in a related field, such as public health or health education, and at least four years of public health or K-12 school nursing experience. Degrees must be from appropriately accredited institutions.LICENSES, CERTIFICATES, & OTHER REQUIREMENTS
A valid license to practice as a Registered Nurse in the State of Washington is required. Performance of job duties requires driving on an occasional basis, a valid Washington State driver’s license, proof of appropriate auto insurance, and the use of the incumbent’s personal motor vehicle on a regular basis. All required certificates, registrations, or licenses must be maintained in an active status without suspension or revocation throughout employment.REQUIRED KNOWLEDGE, SKILLS, & ABILITIES
Knowledge of: Maternal and child health (pregnancy, labor and delivery, postpartum care, and infant and child development). Professional nursing principles, practices, and techniques. Community health issues and inequitable access to healthcare. Epidemiological analysis and surveillance methods and concepts. Motivational interviewing and trauma‑informed care to provide individualized support for clients navigating behavioral changes. Nurse‑Family Partnership philosophy, goals, and core model elements. Community resources related to health and social services (e.g., WIC, TANF, housing, and domestic violence support). Skill in: Analyzing, compiling, recording, and assessing data (e.g., nursing plans of action, medical records, etc.). Motivational interviewing Active listening and interpersonal communication to connect with diverse, vulnerable populations. Asking questions in ways that enhances clarity, quality, and reliability of information Ability to: Establish and maintain rapport with clients. Write in a clear, concise, and organized manner for the intended audience. Proficiently and accurately operate office and other equipment standard to the area of assignment. Maintain emotional regulation during disagreements, seek clarity rather than defensiveness, and show measurable growth through consistent follow‑through. Demonstrate openness to feedback, curiosity about personal impact, and the ability to translate guidance into behavioral changes.
