Heritage Home Healthcare is looking for a Registered Nurse (RN) who wants meaningful patient interaction without hospital shifts, bedside ratios, or routine skilled nursing procedures.

This is a flexible, part-time community-based nursing role providing supervisory visits to an assigned caseload of approximately 25 clients . Visits are generally required every 60 days, giving you greater independence and flexibility in managing your schedule.

$33.00/hour + mileage reimbursement at the current IRS standard mileage rate.

What You'll Do

As an EPSDT RN, you'll conduct face-to-face supervisory visits with clients in their homes and help ensure they're receiving appropriate, high-quality care.

You'll:

• Conduct required supervisory home visits approximately every 60 days
• Manage an assigned caseload of approximately 25 clients
• Assess clients' overall well-being and identify concerns or changes
• Communicate with clients, families, caregivers, and the care team
• Provide education, guidance, and appropriate resources
• Complete accurate and timely visit documentation
• Communicate concerns and recommendations to appropriate team members
• Maintain accurate client records
• Ensure visits and documentation meet applicable requirements
• Work independently while staying connected to a supportive clinical team

Where You'll Work

Most clients are located in Los Lunas and surrounding communities , with a smaller number located throughout the greater Albuquerque service area.

Occasional visits may extend approximately:

• North to Santo Domingo Pueblo
• East to Moriarty
• South to Socorro
• West to Grants

Because this is a community-based position, candidates should be comfortable driving throughout the assigned territory.

Mileage is reimbursed at the current IRS standard mileage rate.

What We're Looking For

• Current, valid New Mexico Registered Nurse (RN) license
• At least 1 year of nursing experience
• Valid driver's license and reliable transportation
• Strong assessment and documentation skills
• Excellent communication and interpersonal skills
• Ability to work independently and manage a flexible schedule
• Comfortable traveling throughout the assigned service area

Experience in home health, community health, case management, pediatrics, or a related setting is preferred but not required.

Nursing Experience Without the Traditional Nursing Schedule
