OASIS Registered Nurse

Hartford Healthcare seeks a compassionate OASIS Registered Nurse for our HomeCare team. In this role, you will provide skilled nursing visits in patients’ homes, completing comprehensive OASIS assessments, developing individualized care plans, and coordinating with an interdisciplinary team. You will manage medications, perform wound and IV care, educate patients and families, and ensure accurate, timely documentation.
