Job Summary The Ward Clerk provides clerical support to the health care team, patients, families and visitors of John H. Stroger, Jr. Hospital. The role contributes to the unit’s efficiency and communication, promoting a positive customer‑focused environment. Typical Duties Receives, greets and directs patients, visitors, and hospital personnel providing accurate responses in a friendly and courteous manner. Checks visitors’ passes. Receives, places and transfers telephone calls using good telephone etiquette. Records, communicates and delivers messages accurately and timely to the appropriate person. Provides accurate information in a concise, informative manner respecting patients’ right to privacy. Observes hospital and unit policy in assisting and directing patients, staff and visitors. Communicate with Courtesy Notifies Admissions Office of all admissions and transfers upon arrival. Remains calm, reassuring and able to provide directions during emergency situations, STAT requests and orders. Establishes good rapport and working relationships to foster interdepartmental communications necessary for patient care delivery. Works cooperatively with co‑workers, nursing personnel, physicians and supervisory staff. Pagers and locate physicians and other hospital personnel upon request, including but not limited to ancillary services: Chaplain, Hospital Police, Interpreter, Social Workers and others. Answers telephone and provides general information; refers questions, which cannot be answered, to the appropriate personnel. Notifies appropriate physicians and nurses of the arrival of new admissions and/or transfers. Notifies the Patient Information Desk regarding patients that are on the Visitor Restriction List and in emergency situations (e.g., Code Blue). Notifies Environmental Services to clean patients’ rooms after discharge or death as needed. Performs Clerical Functions Efficiently Maintains appropriate levels of forms, materials and supplies in an organized manner, and keeps prepaid charts for downtime procedures. Maintains patient charts in proper order with labels affixed accurately. Prepares charts for Medical Records upon transfer, discharge or expiration of the patient. Initiates and accurately maintains identifying information on all paper Medical Record forms. Transfers, checks, and notifies Registered Nurse for Advanced Directives and general consents; provides Patient Handbook to patient or family upon admission. Utilizes patient labels/stickers appropriately and accurately as indicated in policies. Maintains and keeps current medical con‑call binder, admission and discharge log book, census worksheets and snapshots as transactions occur. Completes, organizes and records pertinent information on all admissions, transfers and discharge forms with accuracy and in a timely manner. Maintains and completes the Daily/Monthly Activity Log Sheet. Conducts walking rounds at least every four hours and as needed to ensure accurate census, ID band, and vacant beds. Schedules out‑patient appointments through Cerner – Central Appointment System. Maintains accurate daily/monthly statistical reports. Affixes zebra labels to lab, radiology, and other tests or reports to patient’s medical record in a timely manner. Conducts appropriate follow‑up for late trays, late transport, and patients failing to return from tests or procedures. Provides patients with a copy of the Patient Handbook. Checks, completes and forwards valuables to the Custodian/Cashier and places a copy of Clothes & Valuables Form #1174 on the patient’s medical chart upon admission if applicable. Retrieves patient valuables per policy when requested. Ensures patient ID bracelets are correct (correct location name and Medical Record number) and matches census list at shift start, admission, transfer and at shift end. Handoff process – at the end of each shift, communicate to discuss total census, admissions, discharges, deaths, elopements or transfers. Facilitates patient diet orders as requested. Assists in orientation of new personnel and other unit clerks. Sets priorities in work (e.g., admission, discharge, transfer/emergency situations). Prepares medical records for pick‑up by the Medical Record Department and completes census worksheet by end of each shift if discharging from a remote location. Removes all discharges from Cerner System at least 30 minutes after patients have left the unit. Performs Computer Functions Enters all discharges including AMA, abscond and death data into the computer in an accurate and timely manner. Verifies census and reconciles snapshots from Cerner patient printed list at specified times or as needed. Schedules post‑hospitalization appointments as indicated. Performs data entry in Cerner/Tele tracking, OB QS Prenatal System. Enters and/or updates Medical–Surgical Services and color codes when appropriate. Performs routine cleaning of Wyse terminals, keyboard and printer. Reports any computer or printer problems and/or breakdowns to HIS Help Desk, posts work order number on broken items. Replenishes/replaces computer mouses, toner, and other devices or supplies as needed. Checks and reports safety hazards on all equipment, e.g., frayed cords, loose connections. Demonstrate Awareness of Patient / Family Rights Ensures privacy and confidentiality of all patient-related information. Treats patients and families with respect, courtesy and dignity. Other Related Performance Standards Attends and performs annual mandatory training/competencies as required. Demonstrates punctuality upon reporting to assigned/reassigned unit. Maintains good attendance record. Adheres to hospital dress code. Follows emergency procedures, fire drill, evacuation procedures and disaster plan. Adapts to changes in workload professionally and courteously. Cooperates in implementing changes and activities to improve patient care. Performs other duties as assigned. Minimum Qualifications High school diploma or GED equivalent required (must provide proof at interview). Minimum of one (1) year of clerical experience required. Basic computer skills required. Preferred Qualifications Patient care experience. Bilingual. Knowledge of medical terminology. Knowledge, Skills, Abilities Strong interpersonal skills. Prompt emergency response skills. Excellent verbal and written communication skills necessary to communicate with all levels of staff and a diverse patient population. Demonstrates analytical and organizational, problem‑solving, critical‑thinking and conflict‑management/resolution skills. Attention to detail, accuracy and precision. Ability to meet deadlines in a fast‑paced, stressful environment. Ability to make appropriate and timely decisions. Ability to adhere to department policies and standards utilizing best practices, including EHR use. Ability to follow HIPAA standards. Ability to maintain confidentiality standards for protected health information (PHI). Physical and Environmental Demands This position functions in a healthcare environment. The incumbent must adhere to all hospital and departmental safety requirements, including personal protective equipment, hand washing, sanitizing practices, engineering and work‑practice controls, and any other safety precautions specified by hospital policy. Veteran Preference Preference given to honorably discharged Veterans with at least 6 months of continuous service. The candidate must: Meet minimum qualifications. Identify as a Veteran on the application by answering “Yes” to the question “Are you a Military Veteran?”. Attach a copy of their DD‑214, DD‑215, or NGB‑22 at application filing. If multiple copies exist, submit the latest. Coast Guard Veterans must submit a certified copy of separation from appropriate authority. Provide a valid State ID Card or Driver’s License identifying the holder as a Veteran at interview. Failure to attach required documentation disqualifies the Veteran from preference.
