Therapist - Occupational Therapist

Job Description

Job Summary:
Perform occupational therapy services in a home health setting within the Central Valley/Stockton/Modesto area. This position requires evaluating and treating patients to improve their functional independence and quality of life.

Responsibilities:

• Conduct comprehensive home health assessments using Oasis and other clinical tools.
• Develop and implement individualized treatment plans for patients based on clinical evaluations.
• Document patient progress, assessments, and interventions accurately and timely.
• Communicate effectively with patients, families, and healthcare teams to coordinate care.
• Maintain compliance with travel and licensing requirements, including on-call rotations as needed.

Qualifications:

• Hold an active California state license in hand or be in the process of obtaining it.
• Possess a minimum of 1 year of Home Health experience.
• Demonstrate proficiency with Oasis documentation and home health procedures.
• Be BLS certified through AHA.
• Have reliable personal transportation and a valid driver's license.
