Job Summary and Qualifications

We are seeking a Clinical Pharmacist to join our team. As our pharmacist you would provide clinical pharmacy services for all patients and ensure the safe procurement, preparation, labelling, distribution and control of all medications. What you will do in this role:

• Coordinate, develop, implement, and maintain clinical pharmacy programs to ensure services meet the needs of the patient population, medical staff, and community.
• Provide clinical pharmacy leadership and support across all patient care areas.
• Lead, manage, and oversee clinical pharmacy services including antimicrobial stewardship, pharmacokinetic dosing, anticoagulation management, nutrition support, pain management, IV-to-PO conversion, renal dose adjustment, and other pharmacy-driven initiatives.
• Review and verify medication orders to ensure appropriate drug selection, dosage, route, frequency, compatibility, and therapeutic outcomes based on patient-specific factors.
• Monitor medication therapy regimens for contraindications, allergies, drug-drug interactions, drug-food interactions, and appropriateness of therapy.
• Interpret, verify, and accurately enter medication orders into the pharmacy information system.
• Compound, prepare, and dispense medications safely and accurately in accordance with applicable policies, regulatory requirements, and patient age-specific considerations.
• Ensure compliance with all federal, state, accreditation, and organizational standards related to medication management and pharmacy operations.
• Sustain and manage the hospital formulary by minimizing non-formulary utilization, promoting therapeutic interchange programs, and supporting cost-effective medication use.
• Provide timely drug information, clinical consultation, and therapeutic recommendations to physicians, nurses, and other healthcare professionals.
• Effectively communicate and collaborate with physicians and interdisciplinary care teams to optimize patient outcomes and support evidence-based practices.
• Attend and actively participate in multidisciplinary patient care rounds and bedside rounds.
• Address physician consults and provide expert clinical recommendations as needed.
• Complete drug utilization reviews and analyze medication-use processes to identify opportunities for improvement and promote rational drug therapy.
• Monitor controlled substance processes, including dispensing, reconciliation, documentation, and compliance with regulatory requirements.
• Lead the collection, review, analysis, and reporting of adverse drug events, medication errors, controlled substance discrepancies, pharmacy quality indicators, and drug utilization initiatives.
• Support and participate in organizational quality improvement, patient safety, risk management, regulatory readiness, National Patient Safety Goals, Core Measures, and medication-related patient experience initiatives.
• Document clinical interventions, consultations, and pharmacist activities accurately and consistently. Analyze intervention data and provide reports demonstrating clinical and operational impact.
• Assist in the development, implementation, monitoring, and continuous improvement of pharmacy policies, procedures, clinical protocols, and operational workflows.
• Lead and mentor pharmacists, pharmacy students, technicians, nurses, and physicians through education, training, and competency development programs.
• Develop, assess, and document staff competencies on an ongoing basis and implement improvement plans when performance gaps are identified.
• Serve as a resource for pharmacy staff and hospital leadership regarding medication-use systems, regulatory compliance, pharmacy operations, and clinical practice initiatives.
• Support departmental operational needs, strategic initiatives, and expansion of pharmacy services across the organization while promoting a culture of safety, accountability, and continuous improvement.

Qualifications You Will Need:

• Bachelor of Science or Pharm D degree in Pharmacy from College of Pharmacy
• Experience as a pharmacist in an acute-care hospital setting or general pharmacy practice residency preferred.
• Current FL Pharmacist license or license eligible.
• Eligible to sit for North American Pharmacist Licensure Exam (NAPLEX).
• Critical thinking, service excellence and good interpersonal communication skills, ability to read/comprehend written instructions, ability to follow verbal instructions, PC skills.
