BrightStar Care is looking for a dedicated and compassionate Part Time RN Case Manager to provide in-home infusion and injection services to patients in and around the Burnsville area. This flexible position requires availability 2 days a week between 8:00 AM – 8:00 PM, with travel within a 30-mile radius . Ideally, we're seeking someone who would case manage 4-5 clients. Mileage is reimbursed on a case by case basis. Join our infusion team and make a meaningful difference in the lives of those we serve!

Job Responsibilities:

• Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards.
• Collaborate with multidisciplinary team members, partnering pharmacies and providers to optimize patient care and outcomes.
• Educate patients and their families on health maintenance and disease prevention.
• Ensure accurate documentation and compliance with all regulatory guidelines.

Requirements:

• Active RN license in the state of Minnesota.
• Previous experience in a home health or acute care setting is advantageous.
• Exceptional interpersonal and communication skills.
• Committed to delivering high-quality, patient-centered care.
