Our desire is to see individuals reach their full potential, and create lasting change that will stand the test of time.**JOB SUMMARY**The ACT/FACT Mental Health Professional III reports to the Assistant Program Director and provides mental health clinical services, direct treatment services to patients, and case management services to clients with Serious Mental Illness. This position treats high acuity adults at the highest level of need in an outpatient setting and works as a part of the ACT/FACT treatment team in achieving the goals and objectives of the program to ensure quality of care for client participants.ACT/FACT is a community-based, team-based service to support members living with complex and significant behavioral health needs and a treatment history that may include psychiatric hospitalization and emergency room visits, residential treatment, involvement with the criminal justice system, homelessness, and/or lack of engagement with traditional outpatient services.**ESSENTIAL JOB RESPONSIBILITIES**1. Provide high-quality, therapeutic, wellness and recovery oriented, direct therapy services (individual, family, group treatment, case management, and consultation) in the clinic and in the community as necessary to a caseload of clients with Severe Mental Illness (SMI) and co-occurring-diagnosis individuals.2. Provide primarily community/ field-based services with occasional office-based services.3. Document services to meet all clinical program, county, and state requirements, including Medi-Cal/CalAIM requirements, noting significant intervention and progress toward development and attainment of a treatment plan.4. Participate in clinical consultations and provide clinical directions to other treatment staff.5. Follow all reporting requirements for incidents of Child Abuse, Elder Abuse, Adult Abuse, Dependent Adult Abuse, and Tarasoff warnings per California reporting laws.6. Attend and participate in staff meetings and on-site staff training.7. Attend and facilitate daily team meetings to consult and coordinate client care.8. Be available to respond to crisis in the community9. Provide the Program Director/Assistant Program Director with timely reports on any unusual occurrences that might impact the overall program operation or the health and safety of clients.10. Meet monthly key performance metrics, including end of the month deadlines, productivity standards and documentation timeliness expectations.11. Complete the annual training requirements set by the agency and county.12. Will drive on agency business and transport clients as needed.13. Think and act quickly and efficiently in emergencies.14. Physical presence in the office is required.15. Employees are expected to manage their cell phone use so that the Agency cell phone stipend covers all business usage. Any calls not covered by the stipend are to be made on a company landline phone.## Qualifications**ESSENTIAL JOB REQUIREMENTS**1. Licensed in the State of California as a Clinical Social Worker (LCSW), Marriage and Family Therapist (LMFT), Professional Clinical Counselor (LPCC).2. Current certified training in emergency first aid procedures and CPR, preferred.3. Ability to provide direct mental health services to individuals, families, and/or groups.4. Knowledge of mental illness and effective treatment approaches to mental disorder, including evidence-based practices.5. Provide clinical oversight of licensed or unlicensed mental health providers including in the areas of case management, treatment planning, coordinated care, etc. in accordance to standards set by Program Director.6. Provide effective communication and oversight to ACT/FACT team, that consistently meets ACT/FACT fidelity requirements.7. Experience with co-occurring substance use disorders and treatment modalities.8. Must have experience managing crisis and applying crisis intervention skills.9. Must remain knowledgeable of the new developments in the mental health field including changes in laws and regulations.10. Must have strong oral and written communication skills.11. Ability to operate within Electronic Health Records (EHR) system.12. Communicate effectively in written and spoken English.13. Intermediate computer skills in Office 365, Teams, Zoom and other communication/meeting platforms.14. Possess a valid driver’s license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws. Agency insurer authorization is required.15. Ability to pass a pre-employment criminal background check.16. Ability to pass a pre-employment drug screen and TB test.17. Must maintain good relationships with current, future and past employees, co-workers, vendors, third party administrators, and others with whom the Agency is transacting business, and relate to them in a professional manner.18.
