Cambridge Health Alliance's Program of All-Inclusive Care for the Elderly (PACE) is a nationally recognized model of care that serves adults aged 55 and older living with chronic medical and/or mental health challenges. At CHA PACE, our mission to support our program participants in achieving the highest level of independence within their communities and preventing nursing home placement. Our high-functioning interdisciplinary team supports this goal by providing fully integrated medical and behavioral health services, community resources, and home-based care in a collaborative and supportive environment. Position Overview We are seeking a Registered Dietitian II to join our dynamic PACE interdisciplinary team. In this vital role, you will provide comprehensive nutritional assessments and develop individualized nutrition plans that support the overall health and wellness of our participants. The registered dietitian plays a key role in chronic disease management and works collaboratively with the care team to promote participant independence and safety in the community. Key Responsibilities Conduct nutritional assessments and develop tailored nutrition plans for PACE participants Provide nutrition counseling to support healthy eating habits and chronic disease management Collaborate with the interdisciplinary team to integrate nutritional care into each participant's overall plan of care Monitor and adjust nutrition plans based on clinical outcomes and participant needs Participate in team meetings and contribute to holistic care planning Provide home-based visits as needed to support participant goals and safety Qualifications Master's of Science from an accredited institution Valid Massachusetts R.D. license 1 year experience in geriatrics required; 1 year experience in clinical or community nutrition preferred 1-2 years in ambulatory setting strongly preferred Ability to transport between sites Pay Range: $30.57 - $46.11 per hour Location: CHA East Cambridge Care Center #J-18808-Ljbffr
