Job Summary:
The Clinical Nurse I role provides direct patient care in accordance with the Nurse Practice Act to assigned patients in the Home Health and/or Hospice setting.
The nurse collaborates with other disciplines of the health care team to ensure the delivery of safe, timely and appropriate quality patient care.
This includes providing physical assessments, IV, Infusion, nursing interventions, pain management, medication administrations, specimen collection, meeting nutritional needs, and teaching and discharge needs of the patients assigned.
Utilizes the nursing process based on evidence-based best practices to meet patient care needs.
Evaluates and adjusts the nursing care process as necessary with consultation as needed and confers with the appropriate resource personnel when making ethical/legal/medical dilemma decisions.
Performs other duties as assigned.

Skills:
Required Skills & Experience: -Home Health: Requires at a minimum of two (2) years of acute clinical nursing experience, clinical home health care experience is required.
Hospice: Requires at a minimum of two (2) years of acute clinical nursing experience or home health experience.
Preferred Skills & Experience: -Hospice nursing experience is preferred.
Home Health: Requires at a minimum of two (2) years of acute clinical nursing experience, clinical home health care experience is required.
Hospice: Requires at a minimum of two (2) years of acute clinical nursing experience or home health experience.

Preferred Skills & Experience:
Hospice nursing experience is preferred.

Education:
Required Education: -Home Health: Completion of an Associate's Degree in Nursing or higher education.
Hospice: Completion of an Associate's Degree in Nursing or higher education.
Preferred Education: -N/A.
Required Certifications & Licensure: All Clinical Nurse I's must have current licensure as a Registered Nurse in the State of California.
All Clinical Nurse I's are required to have current BLS certification prior to clinical rotation. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Preferred Certifications & Licensure: -N/A.

Hospice: Completion of an Associate's Degree in Nursing or higher education.

Required Certifications & Licensure:
