Develop and implement individualized care plans based on assessment findings, setting goals and outlining interventions. Administer medications and treatments accurately as prescribed, ensuring patient safety and adherence to protocols. Monitor and evaluate patient responses to interventions, adjusting care plans as necessary and documenting progress in the electronic medical record. Collaborate with multidisciplinary teams to resolve patient care issues and ensure coordinated, comprehensive care.
