Description

• Conduct patient evaluations in both inpatient and outpatient settings and create tailored treatment plans
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Engage with nurses, physicians, case managers, and other rehab professionals to maximize patient outcomes
• Guide patients and their families in understanding therapeutic objectives, discharge planning, and home exercises
• Maintain accurate documentation of evaluations, treatments, and patient progress in line with facility standards and regulations
• Join care team meetings and interdisciplinary conferences to promote a cohesive care approach

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
