JAG Healthcare is currently seeking a compassionate, motivated, and team-oriented *Certified Occupational Therapy Assistant (COTA)* to join our rehabilitation team. This position will provide therapy services across our *Marion, Harding, Mansfield, and Shelby* facilities, offering a flexible schedule and the opportunity to work with a variety of residents and interdisciplinary care teams.

*Who This Position Is Great For*

*Responsibilities*

_(include but are not limited to):_

* Assist the supervising Occupational Therapist with screenings, evaluations, re-evaluations, discharge planning, and implementation of individualized treatment plans
* Provide therapeutic interventions under the direction of the supervising Occupational Therapist to improve residents’ functional abilities and independence with daily activities
* Accurately document treatments, resident progress, and clinical observations in accordance with facility and regulatory requirements
* Monitor resident progress and communicate changes or concerns to the supervising Occupational Therapist and interdisciplinary care team
* Collaborate with nursing, therapy, physicians, and other healthcare professionals to provide coordinated, resident-centered care
* Educate residents, families, and caregivers regarding treatment goals, adaptive techniques, and safety recommendations
* Maintain compliance with all federal, state, facility, and professional practice standards

*Skills & Qualifications*

* Current Ohio Certified Occupational Therapy Assistant (COTA) license required
* Current NBCOT certification, if required by the State of Ohio
* Excellent organizational and time management skills
* Strong written and verbal communication skills
* Ability to work independently while collaborating effectively with an interdisciplinary healthcare team
* Skilled nursing or long-term care experience preferred but not required
* Reliable transportation and willingness to travel between assigned JAG Healthcare facilities
