Puget Sound Home Health & Hospice is seeking an experienced full-time Home Health and Hospice Patient Scheduling Coordinator! At least 2 years of medical office experience with scheduling duties Preference given to those with home health/hospice experience as a scheduler Experience with Home Care Home Base strongly preferred This is a fully on-site position, no possibility of remote work Must be detail-oriented and adhere to compliance standards Technical aptitude to learn and master agency software Demonstrate cooperation & provide excellent customer service to patients & staff Ability to multi-task and stay on task in a fast-paced officeExcellent team-oriented, verbal, and written communication skills What does a Home Health Scheduler do? Act as primary home health and hospice scheduler for clinical staff Ensures optimum staffing solutions by maintaining staff demographics, skill sets, availability, and productivity levels Interface with contract, clinical, and office staff with a focus on collaboration and positive working relationships Serves as the primary responder to all inbound calls and inquiries Provides excellent customer service to patients, referral sources, and community contacts Accurately addresses requests for schedule changes, caseload reassignments, recertification of services, new referral scheduling, and all other visit workflow as assigned Communicates effectively and professionally regarding coordination of care expectations Educates and enforces deadlines, schedule updates, and workflow requirements To learn more about Puget Sound Home Health & Hospice, please visit our website at The employer for this position is stated in the job posting.
