PRN - Hospice Care RN

PRN Visit Registered Nurse (VRN) – Hospice Care Status: Hourly | Non-Exempt Legacy Hospice is seeking a compassionate and experienced Visit Registered Nurse (VRN) to join our dedicated team. In this role, you’ll provide vital care and support to patients in their homes, supplementing our RN Case Managers and ensuring timely responses to urgent needs. Perform focused nursing assessments and address acute needs of hospice patients as directed Ensure continuity of care through effective communication with the interdisciplinary team Complete initial and comprehensive assessments of patients and families to determine hospice needs Develop and revise individualized care plans in collaboration with the patient and caregiver Administer prescribed medications and treatments, and educate families on care techniques Assist patients with activities of daily living and promote comfort-focused care Participate in interdisciplinary team meetings and quality improvement initiatives Registered Nurse (RN) licensed in the state of employment Certified Hospice and Palliative Nurse (CHPN) and/or BSN preferred Minimum 1 year of hospice nursing experience required (2+ years preferred) Self-motivated and capable of working with minimal supervision Legacy Hospice is a drug-free workplace.
