Location: Victorville Employment Type: Full-Time Shift: Days Salary: $35-$50 DOE Schedule: Monday - Friday (9:00 AM - 5:30 PM) – with overtime as needed. Job Site: In-person The LVN Care Coordinator is responsible for ensuring that patient care is coordinated and managed appropriately and for supervising clinical personnel to deliver quality home care services. Essential Job Functions / Responsibilities Receives case referrals and reviews patient information to determine home care needs, assigning appropriate clinicians. Reviews and evaluates each case, assessing services provided by clinicians, conducting record review and case discussions, and guiding clinicians toward effective performance. Review patient's clinical diagnosis, medications, procedures and clinical course. Assists clinicians in establishing immediate and long‑term therapeutic goals, prioritizing care, and developing a plan of care. Attends case conference meetings with organization personnel to facilitate coordination of care. Conducts quarterly record reviews and communicates findings and recommendations to the Clinical Director and appropriate personnel. Assists in screening, interviewing, and orienting new organization personnel, and provides recommendations for employment. Assists the Clinical Director in planning, implementing, and evaluating continuing education programs. Assists in formulating, revising, and evaluating policies, procedures, and strategic goals. Complies with accepted professional standards and principles. Participates in public relations and community activities to promote the organization. Promotes a customer‑service orientation among all organization personnel. Performs other duties as delegated by the Clinical Director. Preferred: Nursing degree. At least two years of case‑management experience within the past five years in a home health agency. Knowledge of clinical decision‑making and quality‑management review and evaluation. Compliance with professional standards and practice. Current CPR certification.
